By digitally signing my name below, I as a Guest/Speaker, and with authority for any professional business entity that I seek to promote, agree to the following in return for my participation with the What, When & Why of Exercise for Women 40+ (the EVENT) 2.0: Older, Stronger, Bolder hosted by Voice for Fitness, LLC DBA Flipping50 (Flipping50):

  1. My participation with the EVENT will include the use of my likeness by Flipping50 for educational, promotional, and commercial use related to Flipping50’s business.
  2. I understand that my “likeness” may include my name, voice, appearance, image, words, recording, and title or professional designation.
  3. I understand that I will not receive any compensation from Flipping50 for the use of my likeness, under any circumstances.
  4. As a speaker I can provide a free registration bonus to the host for use a part of the thank you page at summit registration or a gift to be downloaded with purchase of the fast action package.
  5. I also acknowledge that efforts to promote the summit in which I am participating will provide a benefit to both myself and Flipping50, its hosts, sponsors and affiliates, and I will therefore use by best efforts in promoting the summit with personal emails and social media posts to my network of contacts with the promotional material created by Health Means during the agreed-upon promotional period April 8-May 5, 2024.
  6. With this understanding, I grant Flipping50, including parent companies and subsidiaries, the use of my likeness, including the release of my rights of publicity, privacy or pre-approval for my likeness, to produce, publish, edit, copyright and distribute materials with my likeness in its original form and for the purposes it was created.
  7. I further agree not to re-publish summit materials or to attempt to license, assign or sell any materials generated by Flipping50 without prior written consent.
  8. I expressly release Flipping50 from any and all claims arising out of the use of the material and my likeness granted in this Release and agree to indemnify and hold harmless Flipping50 from and against all claims, losses, expenses, and liabilities of every kind, including reasonable attorney’s fees arising out of the inaccuracy or breach of any provision of this Release.
  9. This release shall be governed in accordance with the State of Colorado. This release constitutes the parties’ entire agreement, supersedes any prior agreement or release, and may be amended only in writing.

By completing and submitting this form, I confirm that entering my name in the fields below acts as a digital and real representation of my actual signature, and confirms that I understand and agree to all terms and conditions stated above and confirm the accuracy of the information provided.
